Job Summary
Full or Part time Psychiatric Nurse Practitioner position available in our Arkansas Valley Region in La Junta, Colorado. The primary responsibilities of the position is to provide safe, quality and culturally sensitive medical services to our patients who seek services. Provider shall initiate and respond to coordination of care activities on behalf of patients with interdisciplinary care team members, including psychiatrists, nursing staff, health navigators, primary care/behavioral health/substance use providers, case managers, and other healthcare personnel to develop, implement, or evaluate treatment plans. Shall offer or refer to medication-assisted treatment (MAT) program, as appropriate and as directed. Provide psychiatric services to acute treatment unit (ATU), as directed. Shall provide or refer to behavioral health groups with a focus on patient needs, such as improving health status, medication management, nutrition, and chronic disease care, as appropriate and as directed. Direct or provide services through approved alternative types of clinical encounters, which may include in-person, telephone/video chat, secure instant messaging, group visits, shared medical appointments, and home visits.

Required Experience & Skills
• Colorado licensed Nurse Practitioner with experience working in a clinic setting
• Bilingual skills (English/Spanish) desirable.
