A Registered Nurse identifies, implements, and evaluates nursing interventions for a given patient population.
MUST BE: Licensed as a registered nurse in the state of PA. Possesses a minimum of (1) year of clinical experience within a hospital, rehab or long term care setting.
General Responsibilities and Accountability
1 Provides home care nursing visits assigned.
2 Submits completed documentation of the home care visit within the established time parameters.
3 Attends multidisciplinary patient care conferences as requested.
4 Instructs patients and caregivers according to the plan of care.
5 Admits patients and develops plan of care adhering to the Medicare criteria for home care provisions.
6 Makes nursing assessments and reports significant changes in the patient’s condition to the appropriate team members.
7 Communicates to physicians any changes in patient’s condition.
8 Obtains physician orders for changing patient care needs.
9 Revises the plan of care based upon assessment findings.
10 Exhibits good written and verbal communication skills.
11 Maintains accurate and current drug profiles for patients assigned.
12 Recommends discharge of patients as appropriate.
13 Follows policies and procedures of Continuous Home Care and secure assistance from Supervisor/Management when needed.
14 Provide management with required licenses and other documents as per JCAHO and the state of PA.
15 Be oriented to EMR (Electronic Medical Record, KINNSER) especially upon data entry, timely document completion and HIPPA patient confidentiality.
Education and Licensure
Licensed as a registered nurse in the state of PA. Possesses a minimum of (1) year of clinical experience within a hospital, rehab or long term care setting.
Competency and Skill Requirements
Excellent written and verbal communication skills. Able to utilize the telephone for internal and external communications. Knowledgeable of nursing assessment skills. Competent in the provision of nursing care.

Full Time Benefits
Working Full Time status (32 points/week) with Continuous Home Care offers:
· Ten (10) paid vacation days accrued at a rate of 0.84 per month and are available after the three (3) month probationary period
· Six (6) sick days that are accrued at a rate of 0.5 per month and are available after the three (3) month probationary period
· Six (6) holidays (New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving and Christmas Day). CHC staff will be compensated at time and a half of the per diem rate below for assigned work on holidays.
· Any hours worked over full time (32 points) will be compensated at the per diem rates. · Health benefits are available after the three (3) month probationary period on the first (1st) of the month.
· 401K benefits are available after working 1000 hours in the first worked year upon the employee’s one-year anniversary of hire date.
Part Time Benefits
Working Part Time status (16 points/week) with Continuous Home Care offers:
· Three (3) paid vacation days accrued at a rate of 0.25 per month and are available after the three (3) month probationary period
· Three (3) sick days that are accrued at a rate of 0.25 per month and are available after the three (3) month probationary period
· Three (3) holidays of your choice (New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving and Christmas Day). CHC staff will be compensated at time and a half of the per diem rate below for assigned work on holidays.
