Liberty Resources provides services and expertise in integrated medical care, behavioral health, and social services to create a whole-health plan individualized for each person we serve. We have an exciting opportunity for a Director to lead a team of primary care practice professionals in our Upstate New York Integrated Family Health Care Center. This position is based out of our newly renovated and expanded Syracuse outpatient health center (with several site locations) where we provide a range of services and supports, including Behavioral Health, Physical Health, Care Coordination, On-Site Pharmacy and Telehealth Counseling. Our collaborative, multi-disciplinary team of licensed professionals all strive for the highest standard of excellence.
This position, working closely with the Vice President, Medical Director and Clinical Directors, oversees the daily operations of the primary care clinic and works collaboratively where the primary care clinic is integrated with behavioral health services. This position supervises the primary care team of clinical and administrative staff.
- The Director will support the vision for and be a role model to implementation of the integrated primary care model. This engaged and collaborative leader oversees the primary care and support functions including doctors, nurse practitioners, nurses, medical assistants and patient access staff. This leader will manage daily operations for Primary Care and collaborate with Behavioral Health to ensure workflows support patient care delivery, including safe and adequate staffing, managing daily operational issues as they arise, ensuring adherence to policies and procedures and promoting the patient experience.
- The Director will lead various efforts for Integrated Health including establishing benchmarks for care outcomes, operations and efficiency of the Electronic Health Record and data reporting/outcome measures, customer care/service, the Tele psychiatry program, quality improvement programs, after-hours support, vendor relationships, physical plant projects and issues, and Integrated Health policies and procedures.
The successful leader will have Bachelor’s degree in a healthcare or business-related field, and must be a NYS licensed Registered Nurse. A Master’s in Public Health or Administration is preferred, with a minimum of 3-5 years primary care healthcare office management experience. This person will have demonstrated leadership skills. Experience in an Article 28 licensed facility and behavioral health setting is strongly preferred.
